What Is a Cochlear Implant?

A cochlear implant, sometimes referred to as a “bionic ear”, is a medical device that provides direct electrical stimulation to the hearing nerve. Unlike hearing aids, which amplify sound, cochlear implants bypass damaged parts of the inner ear and send sound signals directly to the auditory nerve.

Cochlear implants have transformed the lives of many children born with severe to profound hearing loss, allowing them to hear sounds and develop speech and language skills. 

How Is a Cochlear Implant Different from a Hearing Aid?

  • Hearing aids make sounds louder and rely on the inner ear to process sound
  • Cochlear implants convert sound into electrical signals that directly stimulate the hearing nerve

Children who do not benefit sufficiently from hearing aids may be considered for cochlear implantation.

Why a Child May Need a Cochlear Implant

Cochlear implants may be recommended for children who:

  • Are born with severe to profound hearing loss
  • Have significant hearing loss in one or both ears

What Does a Cochlear Implant Consist Of?

A cochlear implant has two main components:

Surgically placed under the skin and into the inner ear

Worn behind the ear and includes a microphone and speech processor

Together, these components work to capture sound, process it, and send signals to the hearing nerve.

Cochlear Implant Surgery

Cochlear implant surgery is performed under general anaesthesia and typically takes a few hours.

During the procedure, the surgeon:

  • Places the internal implant under the skin behind the ear
  • Inserts the electrode array into the cochlea

The surgery is generally safe and well tolerated in children. Most children are discharged within a short hospital stay.

What Happens After Surgery?

Hearing does not start immediately after surgery.

  • The implant is usually switched on a few weeks after surgery
  • The external processor is programmed and adjusted over multiple sessions
  • Sounds may initially seem unfamiliar or mechanical to the child

Over time, the brain learns to interpret these sounds meaningfully.

Will My Child Be Able to Hear and Speak?

With early implantation and consistent rehabilitation, many children with cochlear implants can:

  • Detect sounds
  • Understand speech
  • Develop spoken language
  • Attend mainstream schools

Some children with cochlear implants have gone on to perform as well as — or even better than — their normal-hearing peers. Outcomes depend on factors such as age at implantation, consistency of therapy, and family support.

The Importance of Speech and Hearing Therapy

Cochlear implantation is not a one-time solution. Ongoing support is essential.

After implantation, children usually require:

  • Speech and language therapy
  • Auditory-verbal therapy
  • Regular implant adjustments (mapping)

Parental involvement plays a crucial role in helping children achieve the best outcomes.

Are There Risks with Cochlear Implants?

As with any surgery, there are potential risks, although serious complications are uncommon. Your ENT specialist will discuss these in detail, including surgical and long-term considerations.

When Is the Best Time for Cochlear Implantation?

Early implantation, especially in young children, is associated with better speech and language outcomes. This is because the brain develops sound and language pathways most rapidly in early childhood.

However, older children and even adults with severe hearing loss can also benefit from cochlear implants.

Living with a Cochlear Implant

Children with cochlear implants can lead active, independent lives. They can:

  • Play sports
  • Attend school
  • Participate in social activities

The implant becomes part of daily life, much like wearing glasses or braces.
