What is Ear Wax

Ear wax, also known as cerumen, is a natural substance produced by glands in the ear canal. It helps protect the ear by trapping dust, debris, and bacteria, and usually clears itself without intervention.

The human ear is designed to be self-cleaning, where ear wax slowly migrates out of the external ear canal onto the skin of the outer ear (pinna). However, in some children and adults, ear wax does not clear properly and instead builds up and becomes impacted, leading to blocked ears and hearing problems.

This is more likely to happen when:

  • The ear canal is narrow or curved
  • Ear wax is hard, dry, or produced in excess
  • Ear buds, hearing aids, or earplugs push wax deeper
  • Cotton buds are used, which compact wax further into the ear canal

When ear wax cannot migrate out naturally, it becomes impacted ear wax, one of the most common and easily treatable causes of hearing loss.

What Are the Symptoms of Impacted Ear Wax?

Impacted ear wax can affect both children and adults. Symptoms may develop gradually or suddenly, depending on how quickly the wax builds up.

Common symptoms include:

  • Reduced or muffled hearing
  • A blocked or “full” sensation in the ear
  • Ear discomfort or mild pain
  • Ringing in the ear (tinnitus)

Hearing loss may occur suddenly, such as after swimming when ear wax absorbs water and expands. In other cases, hearing may slowly worsen over weeks or months as wax gradually blocks the ear canal.

How Is Impacted Ear Wax Removed?

There are several methods for ear wax removal. The safest and most effective approach depends on the age of the child, severity of blockage, and ear anatomy.

Ear Wax Softeners

Commercial ear wax softening drops may be used to loosen wax and encourage natural clearance. These should be used with caution, especially in children with:

  • Known allergies to ear wax softeners
  • A history of ear infections
  • A suspected hole in the eardrum

Wax softeners alone may not be sufficient if the wax is hard or deeply impacted.

Professional Ear Wax Removal

If ear wax does not clear with softeners, professional ear cleaning by an ENT specialist is recommended.

At an ENT clinic, ear wax is removed under direct microscopic vision using:

  • Gentle suction
  • Fine, specialised instruments

Why Cotton Buds Should Be Avoided

Cotton buds do not remove ear wax effectively. Instead, they:

  • Push wax deeper into the ear canal
  • Increase the risk of wax impaction
  • Can injure the ear canal or eardrum

Using cotton buds is one of the most common reasons children develop impacted ear wax.

  • Complains of hearing difficulty
  • Has persistent blocked ears
  • Experiences ear pain or ringing
  • Has speech delay or inattentiveness linked to hearing issues
